"Removing barriers that impede Elderly and Underserved Populations from receiving comprehensive care"
Center Highlights:
Morton Plant Hospital CCOE serves as a model resource center to integrate and coordinate available community social and health care services to improve women's access to comprehensive care. It focuses on the needs of underserved women in Pinellas County, Florida, home to an unusually large population of elderly women. This hospital-based CCOE program is located in the Turley Family Health Center, about three miles from the Morton Plant Hospital in Clearwater. In collaboration with their community partners, the center is creating an efficient network of supportive and holistic health care.
"Morton Plant Hospital (MPH) possesses a strong record of providing excellent care and developing solid partnerships. MPH has designed an advisory board of representatives from partner organizations and its target populations to ensure that CCOE services are relevant and responsive to the evolving needs of local women."
The Community Center of Excellence at Turley Family Health Center is a premiere resource for women's health needs. Plans are underway to replicate the model in other communities.
